It is a symptom of a much larger issue. Her identity problems are not unlike those of transgenders. They are not men trapped in a woman’s body or a woman trapped in a man’s body anymore than she is a black woman trapped in white skin. These are all people that have had some emotional trauma that causes they to see themselves different than they feel. It is one more expression of narcissism. But in this case they feel different than they look and they refuse to change their feelings so they work to change reality to support their feelings. Sadly it never ends well for them.
